What will I benefit from your home health care agency?
Gold Team Home Health Agency is a state-certified home health services agency with staff members who have been duly background checked, skills assessed and trained to provide you with care programs at home. Our team is licensed, bonded and insured. We only send care professionals to your home who can effectively meet your health management needs.

If my assigned care provider is unavailable, will you provide a replacement?
Yes, we will. We will assign another capable health care staff member to take his or her place. The substitute will be fully briefed on what state of the treatment or care program you have progressed to. We ensure continuity of care even during a change of staff.

Can I request for a particular care provider from your agency?
You will be able to meet your assigned caregiver prior to receiving care. This will be included in the care plan, which you also have control over. However, the care provider that you will request for may have been previously assigned to a different case. Given that, we always have staff on stand-by who are just as skilled and just as capable to provide you with value-worthy home healthcare services at home.

If I enroll into your care programs, will I sign a contract or a formalized agreement?
Our agency will formalize an agreement for method of payment and service rates. If during the provision of the care program you wish to cancel services, you may contact our office to be advised how to go about the process.

I may have concerns about the care services I am receiving and the care provider I have been assigned with. Is there someone from your staff whom I can call after office hours?
Yes. Gold Team Home Health Agency can still be reached beyond office hours. We have on-call staff who will be ready to attend to your concerns. We aim for your total satisfaction in our care services – even during the hours after our office has closed. You can also submit a message online through our website which is available 24/7.
